We offer high quality, person centred support services for adults with a diverse range of learning disabilities, mental and medical health problems.

We provide consistently high levels of professional care in order that those we support can live as normally as possible, where their individuality independence and dignity are respected and upheld. We are able to offer specialist services to adults with complex medical needs including epilepsy, challenging behaviour and autistic spectrum disorders.

To ensure we deliver the highest service to all our clients we will:

• Conduct a comprehensive individual needs assessment.
• Train staff in all aspects of good care provision and ensure they have the knowledge understanding and training to deliver the assessed care plans.
• To involve all outside agencies and professionals when preparing a joint assessment.
• Ensure we provide support and training for special needs i.e. hearing impaired, cultural issues and language difficulties.
• Offer choice and support to improve quality of life.
• Promote inclusion in local facilities amenities and training establishment.

Nelson Park Care pride themselves on offering each individual client with learning disabilities and associated problems the opportunity to live a full and active life. Undertaking new and exciting experiences within a small safe family environment.

Each of our homes are small and support 1 to 5 clients of similar abilities according to their assessed needs. We firmly believe in empowering each individual to reach their full potential and take an active role in the local community.
